Extended Term Accreditation Agrement

The accreditation agreement is required as part of the pre-application for applicants for third and subsequent renewals. Applicants can chose to upload a signed copy of resolution or use the e-signature option in the online application to sign the resolution. The resolution must be signed by an individual with the proper authority.
